If a villian said something like "deal 5 damage to the highest HP hero and 3 to the lowest HP hero" would my following reasoning hold?
1) all simultaneous actions occur in an order determined by the players and trigger one effect at a time.
Bunker is at 30 life and all others are at 29.
Could the players choose to Deal bunker 5 damage, thus turning him into the lowest HP hero to give him the other 3? Or are the targets choosen in advance?

If they are in the same sentence like that, it stands to reason that they would be selected at the same time, but usually it's not. Usually it's "X deals the hero target with the highest HP 3 melee damge. X deals the hero target with the lowest HP 2 energy damage," which would make it 2 separate calculations.

You would determine the order for each individual instance of damage
For the 5 damage Bunker would be the highest, and he would take it.
For the 3 damage Bunker would be the lowest, and he would take it.

I'm pretty sure you need to go in the order given in the sentence, so you'd first choose the hero with the highest hp, and if that made it so that when you chose the hero with the lowest, he was now the lowest, he'd get pasted twice.

Well, the only time I can think of that it's written like that -- in 1 sentence, with the highest first and the second last -- is on Voss's character card. So, 99% of the time, it won't even pop up, even if you do it wrong. But now that I think about it, I think we have always ran it that way.

And of that 5% (or 25%), how often does the damage trigger? And of those times, how often does the target with the highest hit points then have the possibility to become the target with the lowest hit points?
It's only important when it comes up. For some players, that will be 0% of the time.
