Taking Damage: What If I'm the Highest HP Hero AND the Lowest HP Hero?

So there's one hero left standing duking it out with Voss.  On Voss' turn he hits the lowest HP hero and the highest HP hero.  Does that mean that last hero gets hit twice?

I would presume that if you were the Wraith in this situation and had Smoke Bombs out, the damage would redirect to yourself and be reduced by one. So there's that ;).

If that one hero is Unity then you might not have to take both.

Well, along these same lines I have a follow up question:

The hero with the highest HP takes 3 damage, the hero with the lowest HP takes 2 damage.  Four players total, and two of them are tied for the highest HP, and one person has the lowest HP.

Do the two highest HP heroes BOTH get hit as well as the lowest HP hero?

No. In the case of a tie, the heroes get to choose. So, one of the two highest takes the 3 damage.

If the heroes were all close in HP and the first hit causes the hero that takes it to be the lowest HP, they would then take the 2 damage as well.

What if the effect is to damage the 2nd highest HP hero and there are two tied for highest and one lowest. Who gets hit? I never get this one straight.

For each instance of damage you determine who is what 'position' (e.g. who is highest and who is lowest), so when 2 heroes are tied for highest and damage hits the second highest only one of them is highest, heroes' choice. The other is second highest and takes the hit.
