The PEF makes the 5 lowest-HP targets regain 1 HP when a card is played ... does that count targets that are at full HP?
For example, suppose the lowest-HP targets in play are all 5 heroes who are all damaged, and 2 of Unity's mechs, who are not damaged. Do the 5 heroes regain HP (because the Mechs are at full), or does the card try to heal the Mechs and only 3 heroes regain HP?

Yes. It says the five targets with the lowest HP, not the five targets with the lowest HP and are below their maximum. If Unity is out, her mechs waste some of the healing. If you're fighting The Matriarch, that Environment isn't doing anything.
