Setback has a one-shot ("Whoops! Sorry!") that (1) Destroy 1 non-Character card belonging to another Hero. (2) 
You may destroy 1 Ongoing or Environment card  and (3) Add or remove 3 Tokens from your Unlucky Pool.

Can Setback use the first item on that one-shot to remove an infection card from a hero?

I would say no. Although an infection is played on a hero, it is not a hero card. It is still a villain card and belongs to the villain. It's kind of like how when you play Wrest the Mind on the Electropulse explosive, the card still belongs to Visionary, and a Sedative Flechettes would destroy it.

Plague Rat has ownership of the card, it’s simply played next to a hero to deal out effects. So I would say no as well.
