So having unlocked the Prime Wardens, I decided to attempt DW Setback... again... and failed... many times...
I hate this match. I've had no luck at all. Usually I order them Expat, Nightmist, Setback, then Fixer. I try to have fixer do the hey lifting, with expat doing what she can. NM turtles in mistform asap as the damage is hard and fast. Alas I haven't even taken down The Operative...
I have tried abusing Friendly Fire and Crowbars... no love, things go sideways so fast.
Any other advice?

I restarted over and over until Setback got Friendly Fire and Mr. Fixer got Dual Crowbars. That guaranteed the Operative's death on turn one. After that, I just got lucky.
Edit: Sorry, missed that you tried Crowbar Fire. I think you're just going to have to pray for decks stacked in your favor.

Astral Premonition is one of Nightmist's most underrated cards, I think. The Chairman responds well to deck manipulation, especially once The Operative is dead. Rook City makes it a bit harder, but it's still doable.

You might consider using Advanced as that will get the Chairman to flip sooner but it guarantees you'll have a dead Setback using the Friendly Fire and Dual Crowbars combo to take down the Operative.

Yeah this fight is not fun at all.
Other then the Crowbar thing. Cards to keep an eye out for include speed loading and a ammo as that allows you to keep Mr. Fixer Stocked. SMG or Assault Rifle helps Expat keep the Thugs down so does Mr. Fixer and Jack Handle. Aim and Hairtrigger Reflexes should keep the Thiefs and Informants under Control.
Looking up and Suprising Fortune provides a useful engine to get tokens and restore health. Also with the Dual Crowbars and Friendly Fire. A Silver Lining will help undo some of the damage Setback took killing the operative.
Nightmist has tons of utility but requires some consideration for when you use her abilities. She has some pretty useful spells and the Amulet can be used to either double up on Crowbar Damage by bouncing an attack of her. Or you could use to make the gun man finish himself off if you shot him on his way in with HairTrigger and Aim.
If you check the Ridiculous Challenge one shot thread you might be able to get some additional suggestions.
If you can find the original thread for the unlock, there's advice there. I honestly forget some of it, but it said something like, "If Mr. Fixer doesn't start with a tool in hand, restart the game; if Expatriette doesn't start with a gun in hand, restart the game; if the first villain card played isn't 'Jail Break,' restart the game."

I got really lucky and finally managed to pull this off today, so here's my log for whatever it helps: http://pastebin.com/VfZwb8bE
Basically:
I got ridiculously lucky and started off with Friendly Fire as a Risk flop, two sets of Double Crowbars, and Amulet of the Elder Gods. I also got Tome of Magic and Jack Handle quickly too, and the environment served up allies soon as well.
So I offed the Operative and poor Setback, Setback mostly milled the villain deck, Expatriette fed Fixer guns (the only really unlucky aspect was that Expatriette was often a bit starved for guns to sacrifice), used Fixer to get rid of mooks, and Nightmist pulled cards out of decks and sometimes helped out a bit in killing things.
I eventually ended up with everyone incapped except Nightmist and the Chairman worn down a bit to 22HP. Setback swapped between milling cards and letting Nightmist draw spells from the Tome, Expatriette was just Tome-drawing, Fixer swapped between killing environment cards and even more Tome drawing. Nightmist mostly used the Amulet to redirect damage to the Chairman, occasionally stopping to feed herself cards with Call Forth or Enlightenment or take out mooks with Oblivion or Scouring Mists when they got too numerous. An off-turn supercharged Mist-Fueled Recovery also helped.
Also I cheated a lot with undos. Mostly in the form of: Setback didn't mill a Prison Break? Undo.
But Mr. Adorable is now mine!

Congrats!
For my part, I didn't even know what the DW setback unlock was until this weekend when I got achievement-happy - I wasn't initally gonna bother because the variant is not a change from worse to better. I got him too, though - with more a little bit of luck which, yes, is what you need. Keys for me were:

the Informant played the 2 Prison Breaks early when they didn't matter.

While Fixer's Grease Gun was out, Nightmist and Expatty went postal on the low-level goons.
2.5) Setback did just enough to stay alive long enough to take facepunches for the team
Even after catching those two breaks, it was still pretty hairy and I was pretty amazed I finagled a win with only Setback dying on the last turn. Fitting, I guess?
Based on a sample size of one, my advice for people looking to unlock this achievement is either hope for Friendly Fire and Crowbars turn 1 or live long enough til both Prison Breaks happen and then Grease Gun into a 4/3 Oblivion afterwards. So… yeah.

My first attempt had the Crowbars/Risky Fire.
And an informant who caused all five underbosses to be in play by the end of the second villian turn.
It was then AGES before i got another game with that combo.

Haitrigegr Reflexes in a GREAT first turn drop, even later. With AIM, it insta-kills the pesky mooks.
I never got Dual-Crowbars my game, but I used Jackhandle instead. It cleared out everything with friendly fire.
But don't be afraid to restart your game if the firts turn drop isn't a prison break.

I won with a late game Prison Break, as the Chairman was in single-digits at that point and the Operative had been down for ages.

F6 Wraith was actually not too bad once I got the hang of it. (There was a bug in the unlock nobody yet knew about at the time, so I did it multiple times trying to figure out what I might have done wrong...) Omni-X makes a remarkably good tank; get out Ablative Plating, add a dash of Reactive Plating Subroutine, eat popcorn. Or bring along Haka and have him eat mooks, that works nicely as a snack as well.

My thought being that the robot can't be corrupted because he is fully logical. Just like organized crime wouldn't corrupt someone like Data from Star Trek TNG
