AZ targets Legacy. Setback expends tokens to redirect. Later on same turn AZ targets himself. Setback doesn't have enough tokens, so get hit.
Why? I thought WT&P only affects the first time damage would be dealt to a hero.

Nevermind. I realize my mistake. WT&P triggers FOR EACH HERO.
Really? I'm surprised! I would have thought it would only trigger the first time any hero is dealt damage since the wording is identical to Provitor Tarnis.
The wording is a little different than Provocator Tarnis but I had been under the same impression originally. The wording on Tarnis this

In Setback's case it also makes more sense as only having to deal with only the first time any hero was dealt damage each turn makes Wrong Time and Place a fairly safe play.
I don't see the difference... both effects say "The first time a [target type] would be dealt damage, redirect that damage to [card name]."
In both cases the ambiguity arises from the meaning of the word "a" which can mean "each" or "any". If this card is meant to be "each" I think it needs errata.
Provocator Tarnis with its wording only cares about the first time any other villain target than itself takes damage which is why it only triggers once. Wrong Time and Place is not worded in that it cares about the first time any hero target would take damage.

I feel like I'm not reading the same words you are.... are you saying the phase "other than this card" somehow modifies the meaning of the word "a" two words earlier. Why?
Both {targets that are villain targets and are not Provocator Tarnis} and {targets that are hero targets} are both subsets of the set of {all targets}. Both act as the subject of the sentence which is otherwise identical save for the object of the sentence {this card}/{Setback}. There is 1 other difference in that Wrong Time and Place has an additional clause to its sentence but that's separated by a conjunction ("or") so it should have no impact on the meaning of the first clause.
I agree with Dandolo. The phrase "a villain target other than this card" and "a hero target" seem to be describing the applicable pool of targets for the card's effects to happen. It doesn't make sense to me for both cards to have different effects.
Going just straight off of words, say Tempest and Unity get targeted by attacks. Tempest is selected first, and this is the first time a hero target would be dealt damage this turn, so Wrong Time and Place should go off. The attack on Unity shouldn't get the option, because it is the second time a hero target would be dealt damage this turn.
If the intent was to have it affect each hero target individually, there should have been the word "each" somewhere in there.
Wrong Time and Place tracks the first time each turn that each individual hero target would be dealt damage (similar to how Combat Stance works).
I've branched the discussion off so this can focus on Wrong Time and Place. Please post thoughts on wording at the new location.
If Setback would be dealt damage in the video game (i.e. he is the original target of the damage) he cannot remove tokens from his unlucky pool to redirect that damage to a target. Is this a bug or how wrong time and place is supposed to work? It seems like Setback should be able to redirect (if he has the tokens) since Setback is a hero target.
Can Synaptic Interruption redirect damage to Tachyon? What about Nightmist's Amulet? (I'm not being retorical here, I actually don't know)
If they can't, then presumably it is impossible to redirect damage to the original target, and Wrong Time and Place can't activate because it's trying to do something impossible. That said, if that is the intended behavior, it should be errataed (at least in the video game) to read "other hero targets" because that cleans up the ambiguity without adding significant verbiage.
If it was the first time that turn Setback was taking damage you should be given an option to redirect if you have enough tokens. Do you have the game log by any chance?
It works on Setback, the first time each turn.
My mistake, it was not the first damage dealt to Setback that turn.
